If you would like to be considered for membership in Angel Paws please complete the following. Your responses, along with your evaluation, by a mentor, will be taken into consideration when determining your suitability for our program. Your response to this survey is the first step in the application process. By now you will have passed your evaluation for certification into Delta Society or you are already Delta Certified

 

Steps to become a Dog Member of Angel Paws

* Send in the application to the address given

* Once we have received you application you will be sent a packet. This packet contains :

* Registration for the Providence Orientation given every Monday. Once you have sent the filled out form a Providence person will contact you to make arrangements to attend.

 

* Contact information to receive you TB test.

* Rules of Angel Paws

* Grievence Policy

* Scheduled meeting dates

* Members list

* You will be expect to sign some of the paperwork to be handed to your mentor on your first visit.

* Once you have completed the above you will be given a mentor who will help to famliarize you with Angel Paws, Providence Hospital , as well as instruct you on how to conduct your visits.

* There are a minimum of 3 mentoring sessions. Once the mentor feels that you are ready the mentor will contact the board and give them the paperwork to review. Mentoring will serve as the final review to determine if you and your dog possess the attitude, behavior, professionalism required to be members of Angel Paws. Remember that as a member you are representing Angel Paws, Delta Society, and the Providence Health Care System and that the focus of our group is on therapy, not your dog and certainly not you.

* If everything is in order you will be welcomed into the organization.

Steps to become a Non-Dog Member of Angel Paws

*If you are joining as a Non- Dog member you are expected to join Delta as a non-dog member.

*You will be expected to completed every step just as future members with dogs.

*You will be mentored by a member without a dog.
